Welltower Expands DFW Portfolio with Fort Worth Nursing Care Facility Acquisition

A national healthcare real estate investment trust has expanded its footprint in North Texas with the acquisition of a Fort Worth nursing care facility, adding another healthcare asset to its growing regional portfolio.

According to a Tarrant County deed record filed on July 8, 2026, TGR Holdings Fort Worth, LLC sold Immanuel's Healthcare at 4515 Village Creek Road in southeast Fort Worth to Welltower. The deed was signed by Brian Thomas, President of TGR Healthcare, on behalf of the selling entity. Tarrant Appraisal District records value the 32,398-square-foot nursing care facility at approximately $2.8 million.

Immanuel's Healthcare is an 84-bed skilled nursing and rehabilitation center providing around-the-clock nursing services, rehabilitation, complex wound care, diabetes management, respiratory therapy, and enteral nutrition. The facility serves residents requiring both short-term rehabilitation and long-term skilled nursing care.

Brian Thomas has more than 38 years of experience in long-term care and is a licensed nursing facility administrator and co-owner of TGR Healthcare, a company that manages skilled nursing facilities throughout Texas, including Glen Rose Nursing & Rehab. In addition to his healthcare leadership, Thomas serves as President of the Glen Rose Chamber of Commerce, Chairman of Lake Granbury Medical Center, and sits on several healthcare and community nonprofit boards, reflecting his longstanding commitment to healthcare services across North Texas.
